Paralytic Versus "Non-Paralytic" Polio: A Distinction without a Difference?

چکیده

Non-paralytic polio (NPP) is commonly thought to be synonymous with "abortive polio," in which the poliovirus neither entered the CNS nor damaged neurons. Described are two epidemic illness -"The Summer Grippe" and Iceland disease -apparently caused by a low virulence but neuropathic type 2 poliovirus, studies showing that neuronal lesions in the brain and spinal cord and muscle weakness were common in NPP, and epidemiologic studies documenting late-onset weakness and fatigue in 14% to 42% of NPP survivors. Clinicians should not require a history of paralytic polio, EMG evidence of denervation and new muscle weakness for a diagnosis of "Post-Polio Syndrome," but should be aware that NPP, and possibly even a poliovirus-induced "minor illnesses," can be associated with CNS damage and late-onset muscle weakness and fatigue.

Challenges associated with the recent outbreaks of wild-type Poliovirus in previously polio-free countries

During the last five years, outbreaks of wild-type polio infections following importation of wild-type poliovirus were reported from 39 countries that had been polio-free previously. The contributions through different host genetic profiles and/or any upsurge in neurovirulence of imported isolates might have been crucial for the sudden outburst of disease. For example, such profiles of adults w...

How to interpret normal electromyographic findings in patients with an alleged history of polio.

OBJECTIVE In some patients with a history of polio, the electromyography is normal, not showing the typical neurogenic signs. The aim of this study was to explain the normal findings in electromyography, especially in paralytic polio. DESIGN Retrospective study. SUBJECTS/METHODS Concentric needle electromyography, macro electromyography (including single fibre electromyography) and neurogra...

Protocol for the investigation of acute flaccid paralysis and suspected paralytic poliomyelitis.

Paralytic poliomyelitis has been nationally notifiable in Canada since 1924. The last case of indigenous wild paralytic poliomyelitis in Canada occurred in 1977, and Canada, along with the rest of the American Region, was formally certified as polio-free by the International Commission for the Certification of Poliomyelitis Eradication in September 1994(1). Despite the elimination of indigenous...

Dynamics Affecting the Risk of Silent Circulation When Oral Polio Vaccination Is Stopped

Silent circulation of polioviruses without poliomyelitis cases could threaten eradication when oral polio vaccine (OPV) use is stopped worldwide. Waning immunity promotes silent circulation by increasing poliovirus transmission from individuals not at risk of paralytic polio. There is limited data on temporal patterns of waning. Accordingly, we modeled a range of waning patterns, scaled from fa...
